Accident that happened due to incorrect tools and no thorough supervision by respective personnel. Got it from e-mail forwarded to me. Just to share with readers out there, always be careful when carry out your duty and don't simply take short cut to do your job.

grinders are lethal you need a face shield,for some reason people don't think grinders can kill you.i had a friend a grinder wheel exploded on him and took off his lower jaw,well almost,he lived to drink again thank goodness.
